- Observed 2 packages of Hot Dogs, 1 package of Bologna, and 1 package of Salami open with no date label. I spoke with an employee about potentially hazardous ready to eat foods, how they should be labeled with the open date and discarded after 7 days. Per the employee the deli meat and Hot Dogs were open on Monday. She labeled the packages during the inspection.
- Observed an employees cup of coffee with no lid or straw on the counter behind hot dog hot holding unit. I discussed personal drinks having a lid and straw and having them stored away from facility food.
- Observed the reach in cooler near the 3 compartment sink with food debris build up on the inside. The handle of the reach in cooler and the freezer near the 3 compartment sink have debris build up. All cabinets under the coffee station, specifically the cabinet under the hand washing sink, have coffee debris build up. Clean and maintain routine cleaning.
- Observed the 3 compartment sink to be leaking. Repair.
- Observed the hand sink by the coffee station without hand soap. All hand sinks should be provided with soap and hand towels/effective hand dryer. Spoke with the facility manager and she stated hand soap will be provided.
- Observed a gap at the bottom of the emergency exit door in the office where daylight can be seen. This presents an entrance for pests and rodents. The door should be sealed so daylight can no longer be seen. Correct y 1/21/16.

- Metal stem therm read 40* in ice water.
- No chlor test kit available.
- Found food debris/grease/scum buildup inside oven.
- No soap at handwash sink in men's restroom.
- Found dumpster lid open.
- Found Chlor sant PPM >200PPM in two wiping pails. Food Mgr diluted sant to 100PPM.
- Found wet mops stored in bottom of mop sink.

- Found hot dogs and turkey meat that had been opened not date labeled in the cooler across from the 3-compartment sink. Potentially hazardous food needs to be labeled by date of opening or 7 days past date of opening. Also need to label brownies.
- Observed soda lines resting on the floor. Store all food/ food contact surfaces 6 inches off the floor.
- Shelving in back room has cardboard being used as shelving. Cardboard is not an easily cleanable surface. Remove cardboard.
- Clean the lime build-up around the nozzles of the tea dispenser. Clean lime build-up on ice machine. Also remove unnecessary items from top of ice machine (bulb, wrench, ect.).
- Clean the following areas; floor in office room under shelving where excessive black build-up has accumulated, under soda syrup lines shelving, corners of floor in office room.
- Found motor oil and other car additives stored on same shelving with coffee filter/ butter substitute/ peanut butter/ jelly. Corrected on site.
