- Observed several lidded personal drinks in coolers through out the facility. The manager moved the personal drinks to the walk in cooler where they placed on a shelf marked'personal'. I discussed personal drinks in the kitchen being labeled when stored with facility food.
- Observed uncovered containers of food in the cook line prep cooler. Cover food in storage to prevent contamination. The manager provided covers for food during the inspection.
- Observed food items stored on top of cardboard in the walk in cooler and on the drying rack. Only use surfaces that are smooth, easily cleanable, and non absorbent for food storage. An employee discarded the cardboard during the inspection. The shelves in the walk in cooler are rusty, repair or replace.
- Observed the sanitizer bucket in the check out area without any sanitizer. The manager provided chlorine sanitizer at 100-200 ppm during the inspection.
- Observed the following non food contact surfaces with food debris build up: the soda cooler, the seal and the shelves in the standing white cooler/freezer unit, the chest freezer by the 3 compartment sink, and the storage racks containing the cans. Clean and maintain routine cleaning.
- Observed the faucet on the hand sink in the check out area to be leaking. Repair.
- Observed debris accumulation under and around the equipment and the 3 compartment sink. Clean and maintain routine cleaning.

- Remove the duct tape from the back side of the cook line prep cooler. Duct tape is not smooth, durable, or easily cleanable.
- The flour shelf, kitchen counters, and inside of the upright fridge by fryers had a build up of debris. Wash, rinse, and sanitize these areas.
- The inside of the smoker had a build up of grease and debris. Wash, rinse, and sanitize it.
- The freezer and cooler door seals throughout the establishment, outsides of bulk flour containers, and outside of the wok line had a build up of debris. Wash, rinse, and sanitize these areas.
- Found clean dishes stacked wet. Allow dishes to air dry before stacking them.
- The floors in the kitchen had some cracked and damaged tiles. Replace them.
- The walls had a build up of debris in the kitchen and above the hand sink behind the front counter. Clean the walls.

- Found sweet and sour chicken and egg rolls in the white upright fridge/freezer behind the front counter at 45-47F. Maintain cold food at 41F or below. There was no temperature documentation on this cooler. The facility was closed yesterday. The manager voluntarily discarded these foods. The fridge was found to be set at the warmest temperature. The gauge was adjusted to a lower temperature setting and the fridge was below 41F at the end of the inspection.
- Observed several employees washing their hands for only a couple seconds and then turning off the faucet with their hands, recontaminating their hands. Inspector immediately re-trained all staff on proper hand washing and proper hand washing was demonstrated by each employee.
- Paint or replace rusty shelves in dry storage area. Remove duct tape and paint the rusty edge of the white chest freezer lid across from the 3-compartment sink.
- The insides of the reach in coolers throughout the facility had a build up of debris. Clean and sanitize them.
- The cooler and freezer door seals on all units had a build up of debris. Clean and sanitize them.
- Repair or replace the leaky faucets at the front line hand sink and the 3-compartment sink.
- Replace the cracked floor tiles on the wok line and missing tile under the kitchen prep sink.
- Found build-up of debris on floors. Clean the floors, especially under and around equipment.
- The walls had a build up of debris around the front line hand sink. Clean the walls.

- The non working walk in freezer and storage closet have a large accumulation of unused items. Clean and organize these areas. Remove all unnecessary items.
- Found mops and brooms stored on the floor. Hang them up to dry.
- No current cool down charts were available. Foods were properly labeled with the date and time. Start recording cool down temperatures again and submit a copy to TCHD, Attn: Kim by 6/10/15.
