  • Observed an open soda in the kitchen on a shelf containing food contact surfaces. Employees drinks in the kitchen should be lidded to prevent contamination of items around it and from touching around the mouth piece.
  • Observed the table top can opener with food debris build up. Wash, rinse, and sanitize.
  • Observed the handle on the black cooler in the kitchen broken and dirty. The shelves next to the grill line have food debris build up. There is a build up of grease in the hood of the grill. Routinely clean and maintain routine cleaning.
  • Observed cracked floor tiles in the kitchen area. Repair.
  • Cool down is not being documented. I discussed cool down with the cook. Please send me 2 weeks of documented cool down by 3/23/16.
  • There was no available certificates for food manager or food handler at the facility. Per the manager, all the staff has taken the food handler course and two of the employees have their food managers. Please create a document that contains the employee, their hire date, and the date the food handlers course was taken and keep a copy a the facility. Send document to me by 3/23/16.

  • Clean and remove the dust from the top of the stainless freezer in the kitchen and the vent behind this unit.
  • Finish replacing damaged and missing floor tiles in the kitchen, hallway, and dry storage areas. Tile was on site already.
  • Provide better lighting in the dry storage area by the restroom.

  • Observed a large tube of raw hamburger stored on a shelf with cabbage and pasta salad and above whole tomatoes in the Amana fridge in the dry storage room. Corrected- Hamburger was moved to a lower shelf in another cooler.
  • Observed the dish washing employee shut off the faucet with his hands after washing them in the hand sink, recontaminating his hands. Observed another employee touching his face and chewing a toothpick in the kitchen while cooking. He did not wash his hands. Both employees were retrained on proper hand washing procedures by the inspector. Owner shall conduct a hand washing training with all staff by 8/27/15 and submit documentation to TCHD, Attn: Kim.
  • The shelves in the door of the Frigidaire fridge/freezer in the dry storage room have been repaired with duct tape. Duct tape is not smooth, durable, or easily cleanable.
  • Repair the rusty top of the chest freezer in the dry storage room. Repair the drip tray on under the grill so it doesn't drip on the floor.Repaint the rusty metal cabinets in the kitchen.
  • The cart with the flour store on it in the dry storage room, the inside of the cabinet with the baked good supplies in the dry storage room, the bread racks in the kitchen, and the sugar cabinet in the lobby have a build up of debris. Clean and sanitize these areas.
  • The pipes and conduits throughout the kitchen (under sinks and by the ceiling) had a build up dust and debris. The vent and compressor above the stainless freezer had a build up of grease and dust. Clean these areas.
  • The faucet at the 3-compartment sink was leaking from the handle and faucet. Repair or replace it.
  • There was a gnaw hole in the bottom of the North wall in the dry storage room. Repair it with steel mesh and caulk to prevent the entrance of pests. There were holes in the wall in the hallway between the dry storage room and the kitchen with exposed insulation. Repair these holes to prevent pests harborage areas.
  • There is a gap under the screen door in the kitchen to the outside and a couple small holes in the screen. Repair this door so it seals to the outside to prevent the entrance of pests.
  • There were several (3)small ants inside the syrup cabinet in the lobby area and approximately 5 drain flies hovering around the floor drain outside of the restroom door. Contact a pest control company today and provide receipts to TCHD, Attn: Kim by 8/27/15.
  • Repair the holes in the floor in the hallway between the kitchen and dry storage areas. Replace damaged and missing floor tiles throughout the kitchen, under the stainless freezer, in the hallway, and in the dry storage areas.
  • The floors had a build up of food debris under, behind, and around equipment and coolers in the kitchen, dish room, and dry storage room. There was also a build up of debris behind and around the soda dispensing station in the waitstaff area. Clean the floors. The floor drain outside the restroom had a build up of debris. Clean this drain.
  • Repair the holes in the wall behind the aprons in dry storage area, hole in the ceiling tiles around wires and soda lines being ran through the ceiling, hole in the wall and ceiling above the white chest freezer in the dry storage room, holes in the wall and ceiling in the furnace room, and walls in the hallway between the kitchen and dry storage room.
  • Provide better lighting in the dry storage areas.
  • The chlorine sanitizer in the 3-compartment sink was >200 ppm. Maintain at 50-100 ppm. Corrected to 100 ppm.
  • Found bleach in a bottle labeled Windex in the chemical storage area. It was discarded. Also, found a spray bottle of blue chemical in the chemical storage area. It was labeled by staff as Windex during inspection.
  • Observed Hot Shot insecticide stored with cleaning chemicals in the chemical storage area. Corrected--The pesticides were moved to the office.

  • The plastic drawer in the white fridge/freezer in the storage room that was full of whole tomatoes had a large crack in which was covered with duct tape remove duct tape and repair or replace this drawer.
  • The caulk along the prep table adjacent to the 3-compartment sink was black. Replace it.
  • Found cloth towels on the floor under the grill. Remove them after use.
  • The shelves in the kitchen, flour shelve in the storage room, and fridge and freezer interiors had a build up of debris. Clean and sanitize them.
  • The kitchen microwave interior and the grill had a build up of debris. Clean and sanitize these areas.
  • The pipes throughout the kitchen area, cooler door seals, outside of the grease trap, fronts of the cabinets, and compressor and top of the Traulson stainless freezer in the kitchen had a build up of debris. Thoroughly clean/sanitize these areas.
  • Repair or replace the leaky faucet of the 3-compartment sink.
  • Replace the missing/damaged floor tiles throughout the facility.
  • The floors had a build up of debris, especially under and around equipment. Clean the floors.
  • Repair the hole in the wall behind the door leading to the storage room, replace missing ceiling tile by restroom, and cover the foam ceiling in the storage room.
  • The walls and ceilings in the kitchen had a build up of grease and debris. Clean the walls and ceilings.
  • Found missing light bulb in the storage room. Replace it.

  • Found diced ham, cooked ground sausage, diced tomatoes, hash browns, home fries, and all foods in the grill fridge at 46-53F. Maintain at 41F or below. No temperature charts were available for the last couple of days only, Jan. 17-19, 2015. Manager stated they ran out of copies of their charts last Friday. Food was voluntarily discarded by the manager. Adjust the temperature of this unit to ensure it can maintain food at 41F or below. Do not store potentially hazardous foods in this unit until it is maintaining proper temperature. Contact Kim at the health department for a reinspection.
  • Found cooked sausage patties and links on a tray over the fryers in the kitchen at 88F. Maintain at 135F or higher. Corrected--food was discarded by the manager.
  • Found foods that were being thawed improperly (tube raw hamburger thawing at room temperature in back dry storage room). Potentially hazardous foods must be thawed: in a cooler, under cool running water, or as part of the cooking process. Hamburger was still below 41F and was immediately place in the fridge.
  • Observed duct tape on drawer inside white Amana fridge in storage room. Duct tape is not an easily cleanable surface.
  • Found no chlorine sanitizer in wiping pail in the kitchen. Maintain at 100-200 ppm. Corrected to 100 ppm.
  • Observed food build up inside freezer drawer Amana fridge. Clean and sanitize it.
  • Found damaged/missing floor tiles in the kitchen and the back room. Replace them.
