  • Found open drink in kitchen by food prep counter.
  • Found moist wiping cloth stored on counter.
  • Cutting boards are badly stained & scarred. Also, some plastic food containers & lids are cracked. Found soil/debris in bottom of utensils container. Also, found food debris buildup inside microwave.
  • Found food debris buildup inside microwave.
  • Found dirt/rust/food debris buildup on some kitchen shelves, food containers,equipment, ledges,etc.
  • Found several broken floor tiles in kitchen. Also, floors in kitchen need cleaned & mopped.
  • Found some small screw holes in walls. Also, the ceiling is not smooth or easy to clean above mixer.

  • Observed employee not wash their hands properly. He touched the handle of the faucet to turn it off. The paper towels weren't accessible, so that may be part of the problem. Explained that you must use the paper towels to turn off the faucet otherwise you contaminate your hands. You must verify all employees are properly trained on how to wash hands. I need documentation of this by 10-14-15. The employee also changed gloves, but didn't wash his hands in between glove change. This also must be included in the training.
  • The racks in the reach in cooler of the pizza area are starting to rust and have the paint removed. The seal to the walk in cooler is broken.
  • The sanitizer buckets under the pizza oven is low, 50ppm. The facility's test strips are stained. Suggested getting new strips to be able to accurately test the sanitizer.
  • There is buildup in handles, racks, carts, the microwave, and containers that hold things like flour. The can opener holder on the side of the wood table has buildup in it as well as the actual can opener by the salad area. Inside reach in cooler units it is dirty.
  • There are cups stacked wet behind the bar.
  • Observed an accumulation of debris on floors throughout facility but especially in prep areas and dishmachine area.
  • Observed damaged floor tiles especially in grill area. There is coving missing the by the microwave.
  • Observed buildup on walls especially in dish machine area.
  • The wall is damaged across from the pizza oven. Also there is insulation coming out the side of the pizza oven by the adjoining wall. There are stained ceiling tiles by the pizza area. Also there is insulation coming out over the hot water heater. In the bar area, behind the hand sink there is wood.
  • There was an unlabeled spray bottle on the cart in the dish machine area. It was labeled.
  • No certified food manager was at the facility at the time of the inspection.

  • There were 3 containers of food such as ribs and rib tips in the walk in cooler. Per kitchen employee they were samples from a food distributor. Discussed personal foods. Any foods not going to be served to the public need to be labeled as personal and physically separated as much as possible.
  • The racks in the reach in cooler of the pizza area are starting to rust and have the paint removed.
  • There is a pile of rags under the tv in the pizza area.
  • The sanitizer buckets under the pizza oven were both low ~10ppm. The facility's test strips are stained. Suggested getting new strips to be able to accurately test the sanitizer.
  • The drip tray to the ice machine has pink buildup. There is buildup in handles, racks, and containers that hold things like flour.
  • Observed damaged floor tiles especially in grill area.
  • Observed an accumulation of debris on floors throughout facility but especially in prep areas and dishmachine area.
  • Observed buildup on walls especially in dish machine area.
  • There are stained ceiling tiles by the pizza area. Also there is insulation coming out over the hot water heater.
  • 1 person has gotten their food handler's license. Last inspection I emailed the link to approved online classes and the schedule of classes at TCHD. There was some confusion about how to register for classes. I showed the owner how to get to an online class. I am emailing both again.

  • Observed an employee touching onions with his bare hands. Later he was wearing gloves to the onions. When he washed his hands he touched the handles with his bare hands to turn off the faucet. You must use gloves to touch ready to eat foods and properly wash hands. You must conduct a training not only going over proper glove use but also handwashing. I need documentation of this by 2-11-15.
  • There is duct tape in the salad refrigerator. Also the walk in cooler door handle is broken. There is also tape on the handle of the rolling pin in the pizza area.
  • Observed damaged floor tiles especially in grill area.
  • Observed an accumulation of debris on floors throughout facility but especially in prep areas and dishmachine area.
  • Observed the wall being damaged in a couple of places including by the salad fridge.
  • At time of inspection there wasn't anyone there with their food manager's license. The facility only has 2 employees with their license. Explained that due to being a category 1 facility you must have someone licensed present at all time.

  • Found food items stored in containers in walk in that had dark growth on containers. Ensure all food storage containers are routinely washed, rinsed, and sanitized. Found wine bottles and raw meat stored on rack that had an accumulation of debris and growth. Ensure racks are routinely washed, rinsed, and sanitized.
  • Observed employee handling ready to eat foods without the use of gloves or tongs and without prior handwashing. Observed employee then putting on gloves without handwashing. Ensure no bare hand contact with ready to eat foods. Ensure proper handwashing before glove usage. Observed employee properly wash hands and then put on gloves later in the inspection.
  • The following food contact surfaces were found to have an accumulation of debris: Ice bin bucket and ice scoop, Dough mixer, Knives, Flour container for dough. Wash, rinse, and sanitize these items and surfaces and ensure routine cleaning,
  • The following non-food contact items and surfaces were found to have an accumulation of debris and buildup: All cooling units in kitchen, All racks in kitchen and bar, All carts in kitchen, All shelving units in kitchen and bar, All equipment in kitchen, Soda guns in bar. Wash, rinse, and sanitize these surfaces and items and ensure routine cleaning.
  • Floors throughout kitchen and walk in cooler had an accumulation of debris and dirt. Thoroughly clean all floors and ensure routine cleaning.
  • Walls in facility were found to be cracking and chipping. Walls must be in good repair. Repair.
  • Found accumulation of debris and buildup on walls in kitchen. Ensure walls are thoroughly cleaned and ensure routine cleaning.
  • Found box of handsoap stored with single use food service items. Toxic chemicals shall be physically separate and below foods and food contact surfaces. Handsoap was moved by employee at time of inspection.
  • Kitchen area and bar must be kept free of litter as only necessary articles shall be stored on premise. Remove any unnecessary items from facility.
