Provide ice bath for container to store or store inside refrigeration
Noted several food contact surface in need to sanitize due to excessive acccumulation of food debris: microwave, grills, reach-in units, food prep storage table storing non-food items (phone, meat slicer, book)
Clean, sanitize, and maintain daily.

Noted a couple of live adult roaches at dish-washing area
Pest control is currently provided by Clark Pest Control Service twice a month
Provide proof of service by faxing invoice copies to the health department for the months of August and September.
