- Following items still need to be corrected: Provide single-use paper towels (in a roll for current dispensers) or provide new paper towel holder in both, men's and women's restrooms
- Men's restroom need tissue dispenser
- Provide self-closing and tight-fitting restroom doors
- Provide trash can bags in restrooms (for trash cans)
- Replace missing floor base coved tiles in men's restroom
- Observed other areas of floor base improper installed in kitchen
- Allowed 1 month to fix floor base coving tiles from this reinspection day
- Floor beneath walk-in racks and below kitchen's stove and warewash sink still unsanitary
- Must sweep and mop to remove all trash, food debris, dirt and grease from floors
- Provide Pest Control Service receipt
- Observed rodent droppings around ice machine (backroom)
- Rusted shelves used for kitchen staff locate in walk-in are not approved for food storage
- Remove this shelf from premises and replace with new durable, cleanable rack (such as stainless steel or hard plastic material)
- Obtain chlorine test kit for kitchen staff
- Observed wet and heavily soiled wiping cloths piles up in cabinet below cappuccino machine
- Wiping cloths when unsanitary must be separated in a receptable
- Wet cloths that are still reused must be kept in container with sanitizing solution (such as bleach water) at appropriate concentration (100ppm if using bleach)
- Components of soda fountain nozzels are still unsanitary (observed mildew on nozzels)
- Nozzels shall be removed daily, washed and sanitized appropriately
- All beverage machines still have splashes wilder around nozzel area
- Clean daily
- Remove dirt and spills from area on beverage counter (below and to back of machines)
- Provide light bulbs in walk-in refrigerator
- The lights must work
- They must be shatter-proof bulbs or protected with shatter-proof covers
- Provide protected tongs for jerky
- Provide thermometers for mini-fridge (in kitchen)
- Elevate white pipe over floor sink
- Air gap cannot be less than 1 inch
- Repair sections of walk-in kitchen that are deteriorated (absorbent)
- Surface must be smooth, durable, nonabsorbant and easily cleanable
- Cannot store beverages on ground below shelves in walk-in
- Must keep area below rack clear at least 6 inches for easy cleaning of floor below shelving.

- Kitchen employee and retail (prepackaged food) area employees did not demonstrate basic sanitation knowledge nor proved that facility is maintained clean
- Food safety certified person shall train all food facility employees in basics of food safety, sanitation, food storage, general cleanliness, handwashing, etc
- During first 1.5 hours of inspection, employee didn't use handwash sink was seen cleaning, mopping and moving items around
- Right before bare hand contact with tortilla, kitchen employee was directed to wash hands
- Observed lack of handwashing soap at handwashing sink in kitchen
- Observed paper towels in men's and women's restrooms not in dispensers
- Observed 2 large pots in kitchen's tall reach-in fridge with posole and other with rice
- Both foods were deep in food (4-5 inches) shall rapidly cool potentially hazardous food from 135 degrees to 70 degrees within 2 hours, then from 70 degrees to 41 degrees within next 4 hours using proper cooling method (explained to kitchen employee)
- Observed black grocery bags indirect contact with food on steam table
- One bag was completely melted (such as lining) the metal tray and then rice was place on melted plastic bag
- Steamtable has smell of melted plastic
- Rice was immediately discarded by kitchen employee
- Only materials that are safe and approved for storing or handling food may be used
- No material made of toxic substance(s)
- Observed live and dead cockroaches throughout facility
- Observed some mice droppings next to ice machine (on top of cardboard boxes)
- Premises shall be maintained free of any mice or vermin
- Contact pest control
- Show inspector proof of pest control service during reinspection
- Repeated violation.
