- Observed a dead small mouse on he floor underneath the mop sink
- A live cockroach was observed on the front counter of the facility, as well as some cockroach spotting in the rear of facility
- The mouse was removed from facility
- Pictures were also taken this date
- This facility is hereby closed for vermin problems
- A sign was posted closed
- This facility cannot reopen for business until the vermin problem is eliminated from your food facility and the remaining violations are corrected
- A representative from this agency must conduct a reinspection and give approval for this agency to reopen
- Please contact an exterminating service to assist with this problem
- An unpermitted area has been constructed which is connected to the rear kitchen
- This was being used for food and equipment storage
- Use of this area will be evaluated
- Observed refrigerator temperature to be 45 degrees
- Should be 41 degrees or lower
- Observed discrepancy between food storage labels and the nature of the food
- Properly store food in its designated container
- No available test strips for sanitizing
- Obtain test strips and sanitizing solution immediately
- Noticed unauthorized microwaves and household blenders in kitchen
- Remove or replace with approved certification

- Missing thermometer in one refrigeration unit
- Observed cloth-towel dispenser that was inoperable
- Repair/replace cloth-towel dispenser or paper towel dispenser
- Replace kitchen towel dispenser with approved covering towel dispenser
- Observed old food spoilage in side storage area
- Clean and maintain
