  • Pest control conducted by Terminix on 11/05/09 and building treated for cockroaches
  • Good
  • Wall behind 3-compartment sink has been repaired and all holes and cracks have been repaired
  • Cleaning conducted throughout facility
  • Pest control will be provided by Terminix (according to owner)
  • No visible signs of live vermin on site
  • Bakery is ok to open since imminent health hazard has been corrected
  • Permit is no longer temporarily suspended.

  • Observed handwash station blocked by mop bucket with mop inside
  • Handwash station filled with unclean wiping cloths with buttercream dispenser on top making handwash station inaccessible
  • Remove these items and make handwash station accessible during operating hours
  • Observed cockroach infestation at time of inspection
  • Observed live cockroaches in used/empty flour bags in food preparation areas
  • Observed large live cockroaches coming out of wall in food preparation areas
  • Observed some dead cockroaches of all sizes on several traps throughout facility
  • To fix this problem you need to vermin proof the facility by cleaning, and repairing all gaps and holes throughout facility
  • You will need to provide proof of pest control to rid facility of cockroaches.
