- Observed wiping cloths in bucket but no sanitizer solution
- Maintain/have available sanitizer solution during hours of operation
- Chlorine-100ppm
- Quaternary ammonia, 200ppm
- Observed no hot water at prep sink
- Repair sink so hot water is available at all times, 120 degrees
- Observed food being stored in milk crates in freezer
- Stop using this practice
- Do not use milk crates as food storage
- Observed debris build-up on racks in walk-in fridge
- Clean and sanitize racks
- Maintain racks clean
- Observed cooking pans on floor
- Keep utensils at least six inches above floor
- Observed mold build-up on FRP board behind dishwasher
- Replace FRP board and repair missing coving in same area
- Observed broken tile
- Repair/replace
- Tile has been purchased
- Observed debris build-up under various equipment throughout facility, especially under cooking equipment and ice machine
- Clean and sanitize floors and maintain clean
- All other violations from 12/30/2009 have been corrected
- If violations are not corrected, reinspection fee will be charged to account for that date
- If another reinspection is required for these violations, an office hearing will be scheduled.

- Observed lack of sanitizer used to sanitize surfaces
- \"Real Appeal\" degreaser was being used to sanitize surfaces
- Only use approved sanitizer (chlorine, quaternary ammonia) to sanitize surfaces
- Corrected on site
- Observed lack of warm water at handwash sink and throughout facility
- Highest temperature recorded was 90 degrees
- Make repairs as needed to maintain 110 degrees and above at handwash sink
- No warm water available throughout facility
- Repair so there is water at 120 degrees or higher throughout facility
- This should be corrected as soon as possible.
