- A reinspection was conducted on this date regarding the closure of this facility on 1/31/08 due to a heavy infestation of cockroaches only in the kitchen and storage area
- The receipt from Terminex Pest Control was shown to me
- They serviced this facility on 2/4/08 at 8 a.m
- No cockroaches were observed in the kitchen area
- Many of the cracks and crevices were sealed
- the owner stated that the facility will be served again in a week
- Continue your effort to maintain the premises free of vermin
- This facility is hereby permitted to reopen for business.

- Numerous (10-12) cockroaches both nymph and adult were observed throughout the kitchen including behind the ventilation hood, in the food and equipment storage area and behind the coolers
- This facility shall immediately close until the premises is free of cockroaches
- Seal all cracks, holes and crevices to eliminate harborage
- Thoroughly clean the premises
- It is suggested that a pest control service be contacted
- The fixture is missing from the employee hand wash sink in the meat department replace this fixture
- Discontinue using containers of chemicals for storing produce
- Thoroughly wash the skins of the fruit.
