    Correction: Resurface or replace shelving. Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent.

    Observation: Floor under the stove and door and doorway in the kitchen are not maintained in good repair: Floor is missing tiles, door and doorway has peeling paint.
    Correction: Replace the missing tiles under the stove, repaint the doorway and door to provide a smooth, impervious and easily-cleanable surface. Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: Floor in the back bar is peeling and is not imperious and easily cleanable.

    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: Toilet room doors for the front rest-rooms (2) are not provided with a self-closing doors
    Correction: Provide self-closers for the toilet room doors. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas.

    Observation: Hard to reach places of floors through out the facility (especially under and behind equipment and at floor/wall junctures) and wall behind the steam table in the kitchen, have an accumulation of grime and/or debris.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: Chlorine sanitizing solution used was not at an acceptable concentration: Concentration was below 10 PPM. .
    Correction: Adjust the chlorine sanitizing solution to a level of at least 50 parts per million when the pH is at 10 or less and the water temperature is 100°F.

    Observation: The nonfood contact surface of the following equipment has an accumulation of grime and debris:
    - Hamburger pressing device
    - Door Gasket to the low boy reach in refrigerator in the kitchen.
    - Fan Shroud for the walk in refrigerator.

    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: Dumpster lid observed open.
    Correction: Cover all waste containers when not in continuous use.
  • Floors, Walls, and Ceilings - Cleanability
    Observation: Ceiling in the kitchen prep area is not smooth and easily cleanable.
    Correction: Repair or replace ceiling to make it smooth and easily cleanable. A work order is in to replace the ceiling tiles with smooth ones.
  • Toilet Rooms - Enclosed
    Observation: All toilet room doors are not provided with a self-closing door.
    Correction: Provide a self-closer for the toilet room door. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas. A work order is in to replace all doors. All new doors will be installed with self closers.

    Observation: Wall next to the rest-rooms near the dance floor is not maintained in good repair
    Correction: Repair the wall. Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. A work order is in to repair the wall.
