A work order has been placed to repair the burger freezer box door and damaged tiles on the mop sink. Ensure the hose on the mop sink is disconnected when not in use or install appropriate backflow prevention.

Observation: The door on the hamburger freezer box was observed in a state of disrepair and damaged.
Correction: Repair the door to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the door, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: Light bulb cages and the underside of the top of the fries warmer.
Correction: Maintain nonfood-contact surfaces of equipment clean.

Observation: A top lid on the outside dumpster was observed missing.
Correction: Cover all waste containers when not in continuous use.

Observation: Bulb in the walk-in freezer was not observed to be functioning.
Correction: Ensure all bulbs are functioning to provide at least 10 foot candles at a distance of 30 inches from the floor in the walk-in refrigeration units and dry storage areas and in other areas and rooms during periods of cleaning.

Observation: The wall tile in the mop sink is not maintained in good repair
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
