Complaint received regarding no hot water in establishment. Valve broke on water heater late last night. Plumber intends to replace unit this afternoon. Spoke to owner (Lou) who agreed to lock the doors and stop operating. Do not allow baker to produce donuts until hot water is restored. Once restored you may operate.

New gas water heater has been installed. Baker arrives at 8-10 PM tonight for production. Facility intends to open tomorrow at 5:00 AM. System has not produced 100 F yet. I will call tomorrow AM. Call to manager at 8:15 AM 4/14/15. Hot water has been restored.

Observation: Insect control device is located over clean tray cart in the dishroom where dead insects may be impelled or fall.
Correction: Install the insect control device away from a food preparation area so that dead insects and insect fragments are prevented from being impelled onto or falling on exposed food, clean equipment, utensils, and linens, and unwrapped single service and single use articles. Exposed food and food-contact surfaces must be protected from contamination by insects or insect parts. Installation of the device over food preparation areas or in close proximity to exposed food and/or food contact surfaces could allow dead insects and/or insect parts to be impelled by the electric charge, fall, or be blown from the device onto food or food-contact surfaces. Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

Observation: Quaternary Ammonium sanitizing solution used was not at an acceptable concentration. Quat sanitizer was less than 150 parts per million.
Correction: Adjust the sanitizing solution to 200 parts per million or otherwise identified by manufacturer's recommendations. PIC temporarily corrected by manually adding sanitizer - now 200ppm Quat. Service tech has been called and will repair by noon tomorrow. Employees were instructed to use test strips when sanitizer is added manually to insure proper concentration.
