Observation: Insulated containers found not properly stored to adequately drain/air dry.
Correction: After cleaning and sanitizing, equipment and utensils: (A) Shall be air-dried or used after adequate draining as specified in the first paragraph of 40 CFR 180.940 Tolerance exemptions for active and inert ingredients for use in antimicrobial formulations (food-contact surface sanitizing solutions), before contact with food
- Plumbing System/Maintained in Good Repair (corrected on site)
Observation: The cold prep room hand washing sink drain pipe was found leaking water onto the floor.
Correction: A plumbing system shall be maintained in good repair. Improper repair or maintenance may result in the creation of obnoxious odors or nuisances, and may also adversely affect the operation of warewashing equipment or other equipment which depends on sufficient volume and pressure to perform its intended functions.
- Physical Facilities/Repairing
Observation: The FRP wall paneling at the floor base tiles are not completely sealed in the production room to the right of the hand washing station located by the ovens. The FRP wall paneling inside the chemical storage room is no longer sealed tight to it's adjoining wall. There is some slight wall damage at the driver employee hand washing sink and at the cove base to the right of the toilet. There are a few crevices around the door frame to the right of the dish machine and there are some holes in the wall where a wall rack is pulling away from the wall to the right of the dish machine.
Correction: Physical facilities shall be maintained in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. This requirement is intended to ensure that the physical facilities are properly maintained in order serve their intended purpose.
- Controlling Pests (repeated violation)
Observation: A couple dead roaches were observed on the floor in the dish washing area and inside the ceiling light fixture at the driver locker storage area.
Correction: The premises shall be maintained free of insects, rodents, and other pests. The presence of insects, rodents, and other pests shall be controlled to eliminate their presence on the premises by eliminating harborage conditions.

Observation: The delivery rest-room ventilation fan is no longer working.
Correction: If necessary to keep rooms free of excessive heat, steam, condensation, vapors, obnoxious odors, smoke and fumes, mechanical ventilation of sufficient capacity shall be provided. When mechanical ventilation is necessary, it must have adequate capacity to ensure that soiling of walls, ceilings, and other equipment is minimized

Observation: The walk-in refrigerator #3 interior door is damaged. The delivery rest-room ceiling fan vent cover is missing. There is some wall damage by the lockers.

Observation: Some roach droppings and dead roaches were observed in the hot water heater closet and delivery rest-room/locker area.

Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces. Effective measures must be taken to control their presence in food establishments.
- Closing Toilet Room Doors
Observation: The women's production rest-room toilet room door is not closing completely by itself.
Correction: Toilet room doors as specified under section 6-202.14 shall be kept closed except during cleaning and maintenance operations. Toilet room doors must remain closed except during cleaning operations to prevent insect and rodent entrance and the associated potential for the spread of disease.

Observation: Insect, rodent, and/or other pest harborage conditions exist on the premises. Mouse droppings were observed on the floors around the edges of the walls. A couple of dead American cockroaches were observed in the ceiling light fixtures and one on the floor in the bakery area. A live immature German cockroach was observed on a wall in the bakery as well as some droppings.
Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by eliminating harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces. Effective measures must be taken to control their presence in food establishments.

There were cooling trays of ground beef inside the cold room the beef was found at at 56 F and the temperature of the cold room was 38 F the cooling process started 1 1/2 ago from the time that the inspection started,(no violation). Mice dropping were found inside the bakery room.
