Violations:

• 1570 - The top portion of the ice machine was observed in a state of disrepair and damaged.
  Repair the ice machine to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the ice machine, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
• 1770 C - Observed accumulations of dust, dirt, food residue or other debris on the following nonfood-food contact surfaces: bottom shelf of the Gibson freezer and the nozzle holders.
  Maintain nonfood-contact surfaces of equipment clean.

December 18, 2007 (Routine)

Violations:

• 0520 - Corrected During Inspection Ice used as a medium for cooling the exterior surfaces of food is also for drink ice or used as food.
  Cease use of ice used as a cooling medium for food. Ice that has been in contact with unsanitized surfaces or raw animal foods may contain pathogens and other contaminants.
• 1800 - Observed accumulation of grime on the door gasket of the small reach-in refrigerator.
  Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
• 3045 - A sign or poster that notifies food employees to wash their hands is not provided at all lavatories used by food employees
  Provide a sign or poster that notifies food employees to wash their hands at all handwashing lavatories used by food employees.

December 04, 2006 (Critical Procedures)

Violation: 0820 - Corrected During Inspection Critical Sliced ham and swiss cheese cold holding at improper temperatures. (both items were over 45 degrees. REMINDER: March 01, 2007, all refrigeration / cold holding will need to be 41 degrees and below./
Relocate food to a refrigeration unit that is capable of maintaining food storage at 41F (45F) or below.
Comments:
Please take this as a reminder that the health permit has to be renewed before the 29th of December. The invoice for payment and the sign off sheet were left at the facility at the last inspection. A follow up phone call was made in November.

October 16, 2006 (Routine)

Violations:

• 1190 - Inspector was unable to find a thermometor in the 2 freezers and in the deli top refrigerator..
  Provide a temperature measuring device that is easily readable and accurate to +/-3 degrees Fahrenheit in the intended range of use.
• 1570 - The door gasket of the Beverage Air refrigerator was observed in a state of disrepair and damaged.
  Repair the gasket to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter..
• 1580 - The cutting board(s) along the top of the deli refrigerator is heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
  Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
• 1800 - The nonfood contact surface of the left side deli door gasket (mold); also, the outside of the ice bucket needs cleaning and these items have accumulations of grime and debris.
  Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
• 2000 - Cleaning supplies and other items not stored 6 inches above the floor..
  Keep all items up off the floor for easy cleaning.
• 2920 - Toilet room door is not provided with a self-closing door
  Provide a self-closer for the toilet room door. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas.
• 3030 - No disposable towels were provided at the hand washing lavatory in the bar hand wash sink.
  Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

Comments:
Inspection to renew the permit. Invoice and sign off sheet left with the person in charge. Inspector suggested more shelving (over the sink) to keep items up off the floor.

December 12, 2005 (Routine)

Violations:

• 0610 - Cooking oil stored on the floor or food stored less than 6" above the floor.
  Elevate food storage onto approved shelving with minimum 6" legs or casters.
• 1720 - A sanitizer test kit is not being used to ensure the chemical sanitizing solution(s) are at the proper concentration.
  Use a chemical test kit to ensure the sanitizing solution is maintained at the proper solution.
• 1770 A - Critical The following equipment food-contact surfaces were observed soiled to sight and touch: Cutting board, door gaskets on white under counter refrigerator.
  Clean and sanitize these surfaces for food contact.
• 1780 - Critical Ice mahcine used for storage of packaged or open food items was observed with visible accumulations of soil and debris.
  Clean ice machine at a frequency to preclude accumulation of soil residues to prevent contamination of food in storage and reduce attraction of insects and rodents.

November 10, 2004 (Routine)

Violations:

• 1770 B - Observed accumulations of encrusted grease deposits or other soil on the following food contact surfaces: Bottom shelf of reach in refrigerator
  Clean and sanitize these surfaces for food contact.
• 1790 - The (cavity, door seal) of the microwave oven is observed soiled.
  Using the manufacturer's recommended cleaning procedure, clean the (cavity, door seal) of the microwave oven at least every 24 hours.
