If unable to repair the freezer, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: The door gasket to the Milk box is in poor repair.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.

    Observation: Floors under dry storage shelves noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: The nonfood contact surface of the True (160159) cooler racks observed rusted and peeling.
    Correction: Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent.

    Observation: The door gasket to the Hobart (1841) is in poor repair. Also Crescor warmer upper door will not remain closed.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.

    Observation: Observed food stored on the floor in the dry storage room.
    Correction: Elevate food storage onto approved shelving with minimum 6" legs or casters.
  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: Grilled chicken (43 degrees F) for salads was cold holding at improper temperatures.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.

Discussed observations with the CFM. Observed good food temperature monitoring and records were being maintained. NOTE: The following units had work orders in place: The ice machine and the 2-dr R/I freezer 195335 (this unit only had bread being stored inside during this inspection).
  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: Foods were cold holding at improper temperatures in the 2-dr reach-in refrigerator # 140657. Foods inside the unit were at the following temperatures: Hot dogs 50 F, Chicken patties 67 F, Sliced turkey 58 F. The CFM voluntarily discarded the food product and was going to place a work order for the unit.
    Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below, or discard the food based on time/temperature of the TCS foods. Ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.
  • Temperature Measuring Devices - Ambient Air and Water
    Observation: The ambient air temperature measuring devices (degrees F) located in the #140657 two door reach-in refrigerator were not accurate.
    Correction: Provide a temperature measuring device that is accurate within +/- 3 degrees Fahrenheit in the intended range of use.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The following was observed in a state of disrepair and damaged: The upper door on # 89516 warming unit would not close properly and/or tightly. The two door reach-refrigerator # 140657 was not cold holding at the proper temperature.
    Correction: Repair the above mentioned equipment to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the above mentioned equipment, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

Facility was very clean and well organized. The staff is very knowledgeable of food safety principles. The A/C in the kitchen is not working and should be fixed as soon as possible to prevent any potential heat injuries that could affect the staff.

    Observation: Milk coolers were observed in a state of disrepair and damaged. Both have damaged door gaskets and the milk cooler to the left had some damage at the bottom section which is preventing it from working to its full capability.
    Correction: Repair the @EQUIPMENT@ to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

Recommend the reach in freezer without shelves have shelves installed to allow for proper air flow and to replace melted and frayed hot mitts. Health Permit issued.
  • Cooling Methods (corrected on site) (repeated violation)
    Observation: The methods used for cooling were not adequate. Sealed bag of grilled chicken patties cooked this morning observed in the reach in cooler at greater than 90 degrees.
    Correction: Cool foods by the following methods: 1. Placing food in shallow pans 2. Separating food into smaller thinner portions 3. Using rapid chill cooling equipment 4. Stirring the food in a container placed in an ice water bath 5. Using containers that facilitate heat transfer 6. Adding ice as an ingredient 7. Any other method that can effectively accomplish cooling.
  • Nonfood Contact Surfaces
    Observation: The nonfood contact surface of the scoop handles are melted and can not be easily cleanable.
    Correction: Alter or replace this item to provide a surface that is free of unnecessary ledges, projections, and crevices.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The door gasket to the True reach in refrigerator # 160159 is in poor repair.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.

    Observation: The following equipment food-contact surfaces were observed soiled to sight and touch: Slicer stored in dry storage room.

    Observation: Chemical sanitizer immersion time was not adequate for Quat sanitizing solution.
    Correction: Immerse or expose food contact surfaces to sanitizing solution for adequate time.

    Observation: Floor wall juncture in dry storage room and several areas throughout the kitchen is not coved and closed to no larger than 1/32 inch space.
    Correction: Cove floor wall juncture to no larger than a 1/32 inch space.

    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents. Screen in back door is damaged.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

    Observation: Wall cracks in several areas in the kitchen and in the office is not maintained in good repair
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Floor in corners and under shelves noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
  • Pests - Controlling Pests*
    Observation: Premises are not being routinely inspected for evidence of pests. Catchmaster have collected some cockroaches however mice droppings and damaged food products from mice knawing were observed throughout the dry storage room.
    Correction: Inspect premises on routine basis for the evidence of pests. . Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces. Food products and single service items were voluntarily discarded. Set traps.

Discussed proper cooling methods and taking temperatures during the process. Recommend the back screened door have a latch installed.
  • Cooling Methods (corrected on site)
    Observation: Bag of chicken patties that were cooked this morning were sealed closed and located in a refrigerator that does not allow for maximum heat transfer. Internal temperature of patty was 83F.
    Correction: Arrange the food containers in the refrigeration equipment to provide maximum heat transfer through the container walls. Loosely cover or uncover the food if protected from overhead contamination during the cooling period to facilitate heat transfer from the surface of the food. Chicken patties were placed on a sheet tray.
  • Floor and Wall Junctures, Coved, and Enclosed or Sealed
    Observation: Floor and wall juncture in the dry storage room and kitchen under three compartment sink not sealed.
    Correction: Seal floor and wall juncture in rooms where water flush cleaning methods are used.
  • Outer Openings - Protected
    Observation: Outer opening screen mesh is more than 16 to 1 inches. Back door screen is torn and damaged.
    Correction: Provide outer opening screen with a mesh that 16 to 1 inch. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces. Recommend a screen protector be installed to prevent damages during delivery of supplies or taking refuse out.
  • Physical Facilities in Good Repair
    Observation: Electrical outlet cover mounted on the low wall near the milk cooler on the service line is coming off causing a trip hazard to staff.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
