Facility in process of physical expansion--adding a bar in back of facility. Will need to submit plans to this department for approval.
- Critical: Hands - When to Wash* (corrected on site)
Observation: A food employee failed to wash his or her hands before engaging in food preparation, after touching bare human body parts, after coughing, sneezing, eating, after handling soiled utensils or after engaging in any activity which may have contaminated his/her hands.
Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

Operator is planning to submit plans and equipment specs for expanded dishwashing area and possibly a lounge area in rear of facility with very limited menu. Walls to be resurfaced then.
- Handwashing - Using a Handwashing Lavatory
Observation: The handwashing facility located at the new prep area is directly adjacent to prep sink. The prep sink is exposed to potential splash and contamination from handsink.
Correction: Operator agreed to install a splash guard on right side of handsink to prevent splash on prep sink.
- Wall and Ceiling Coverings and Coatings
Observation: Concrete block wall behind new prep area is not finished and sealed to provide a smooth, nonabsorbent, easily cleanable surface. Also gap at wall juncture of prep sink in new prep area can allow moisture to drip behind sink. Trim on window at prep area is rusting and needs resealing/painting
Correction: Finish and seal (Concrete, Porous block, Bricks) to provide a smooth, nonabsorbent, easily cleanable surface. Also attach prep sink to wall and caulk to prevent moisture entry behind sink.

- Non-Food Contact Surfaces
Observation: The nonfood contact surface of the ice machines has accumulations of grime and debris.
Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
- Hand Drying Provision (corrected on site) (repeated violation)
Observation: No disposable towels were provided at the hand washing lavatory in the @Location@
Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials
