8-15-11, 2nd request to have third party FH cards converted to WCCHD. Stina Hill is the contact person I spoke with about this issue. No demerits. St

No WCCHD food handler cards STILL. Ongoing issue since 2009. Follow-Up required. Told mgr to fax in cards once received.
No hot water. Multiple dented cans on shelf. Blocked sink w/ no paper towels. Drink in deli.
Demerits- 8, 10, 14, 16, & 17. DJ.
