All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Found top of appliances not clean.(oven at lower level/coffee machines)
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. Found containers not stored properly on floors by 3 compartment sink.

Found shelving units not wiped down at lower level preparation areas.

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE FOLLOWING NOT CLEAN- OBSERVED FOOD/DUST BUILD UP ON SHELVES AT PIZZA PREP AREA AND INTERIOR OF CABINET UNDER MEAT SLICER. MUST CLEAN AND MAINTAIN ALL UNCLEAN EQIUPMENT.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED FOOD/DIRT BUILD UP INSIDE FLOOR DRAINS IN PREP AREA. MUST CLEAN AND MAINTAIN INTERIOR OF UNCLEAN FLOOR DRAINS.
The walls and ceilings shall be in good repair and easily cleaned. OBSERVED STAINED/DAMAGED CEILING TILES ABOVE CONVECTION OVEN AT BASEMENT. MUST REPLACE STAINED/DAMAGED CEILING TILES.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. PREP LINE HANDWASH BOWL PIPE LEAKING AND 3-COMPARTMENT SINK PIPE DRAINING SLOWLY. INSTRUCTED MANAGER TO REPAIR AND MAINTAIN SINKS.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. THERMOMETER BROKEN INSIDE SANDWICH REACH IN COOLER. MUST REPLACE WITH WORKING THERMOMETER.
