A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. No City of Chicago certified food handler on premises while potentially hazardous foods (beef, sausage, etc.) are prepared. Instructed manager a City of Chicago certified food handler must be on premises while potentially hazardous foods are handled. Citation issued. Ticket #H000058042 12.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. The following not clean- observed slight lime build up inside ice machine and food build up inside prep cooler door gaskets. Must clean and maintain equipment.
The walls and ceilings shall be in good repair and easily cleaned. Observed water stained ceiling tiles above rear dining room. Must replace all damaged ceiling tiles.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. 3-compartment sink faucet too short, does not reach all 3 compartments. Must provide extention to faucet, must reach all 3 compartments.

All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. FRONT DOOR IS NOT RODENT PROOFED. OBSERVED A 1/4' GAP ALONG THE BOTTOM OF THE DOOR. MUST RODENT PROOF THE DOOR. SERIOUS VIOLATION 7-38-020.
