All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Remove rust from inside the ice machine.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. Over 25 rat droppings were found in the basement perimeter and holes were found under the stairs and in the old cooler insulation. Clean up and remove all evidence of rodent activity.
The walls and ceilings shall be in good repair and easily cleaned. Complete repairing the rear kitchen wall and ceiling so they are smooth and sanitary. Repair the damaged ceilings in the basement so rodent harborage can be eliminated.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Clean the floors under the kitchen sink and stoves. Replace the damaged tile under the fryer.
No evidence of rodent holes or droppings were found at this time.

All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. THE SALAD/SANDWICH COOLER HAS A 54 F AMBIENT AIR TEMPERATURE. THE NORTH BASEMENT COOLER HAS A 65 F AMBIENT AIR TEMPERATURE. The problem coolers were tagged held for inspection until repaired or replaced and inspected by the Chicago Department of Public Health.
All cold food shall be stored at a temperature of 40F or less. All hot food shall be stored at a temperature of 135F or higher. THE SALAD/SANDWICH COOLER IS HOLDING CHEESE, OPEN DRESSINGS, CUT READY TO EAT GREENS AND CUT FRUIT AT 54F. THE NORTH BASEMENT COOLER IS HOLDING CREAM CHEESE AND OPEN SOUP AND SAUCE BASES AT 65F. A PAN OF COOKED NOODLES IS BEING HELD AT 91F ON TOP OF THE STEAM TABLE. TEN POUNDS OF FOOD VALUED AT $52.00 WAS DISCARDED.

THE REAR GARDEN PATIO WERE FOOD AND DRINK ARE SERVED IS ONLY ACCESSIBLE THRU THE MIDDLE OF THE KITCHEN AND REAR GARDEN PATIO PATRONS HAVE NO ACCESS TO THE BATHROOMS EXCEPT THRU THE KITCHEN WHICH HAS NO SEPARATION FROM SAID CUSTOMERS WHO WERE WALKING THRU THE KITCHEN DURING THIS INSPECTION. MUST PROVIDE A SEPARATION BETWEEN CUSTOMERS AND THE KITCHEN PER CITY CODES, SUPERVISOR CONSULTED. THE KITCHEN CEILING AND FURNACE DUCT HAS EXPOSED INSULATION ABOVE THE KITCHEN PREP TABLE, SEAL CEILING SO IT IS SMOOTH AND SANITARY.

MUST USE FOOD GRADE BAGS AT ALL TIMES FOR FOOD NOT IN ORIGINAL CONTAINERS IN THE FREEZERS AND THEY MUST BE LABELED.
MUST DECLUTTER AND BETTER ORGANIZE ITEMS IN THE HOTWATER HEATER ROOM SO AS TO HAVE ALL ITEMS 6 INCHES OFF THE FLOOR AT ALL TIMES.ALSO MUST REMOVE ALL EQUIPMENT NOT IN WORKING ORDER FROM OUTSIDE PATIO SITTING AREA.
MUST CLEAN INTERIOR LID OF ICE MACHINE IN THE BASEMENT.MUST DETAIL CLEAN AND SANITIZE UNDER COOKING GRILLS TO REMOVE GREASE BUILD-UP AND FOOD DEBRIS.
MUST DETAIL CLEAN FLOOR AT CORNERS UNDER ALL HEAVY COOKING EQUIPMENT TO REMOVE GRIME BUILD UP.
MUST REPAIR ALL TORN SEAT COVERS AT DINING ROOM AREA SO AS TO BE SMOOTH AND EASILY CLEANABLE.
