All cold food shall be stored at a temperature of 40F or less. Potentially hazardous food at improper temperatures:30 dozen eggs at 47.3F, 5 gallons milk at 46.9F, 15 pounds cheese at 47.8F, and 30 pounds cucumber sauce at 46.9F. Critical violation. citation issued.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Found counter not wiped down at rear storage areas.
FOUND NO BACKFLOW PREVENTION DEVICES INSTALLED AT 3 COFFEE MACHINES.

THE FOLLOWING NOT CLEAN- OBSERVED GREASE BUILD UP ON VENTILATION FILTERS ABOVE GRILL AND DUST/DEBRIS BUILD UP ON SHELVES BY 3-COMPARTMENT SINK. MUST CLEAN AND MAINTAIN ALL UNCLEAN EQUIPMENT.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MOP SINK COLD WATER VALVE TURNED OFF. MUST TURN ON/REPAIR AND MAINTAIN SINK.
