All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.MUST REPLACE/REPAIR RUSTY SHELVING INSIDE 2 DOOR COOLER USING A NON-TOXIC FINISH.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use.ALL SINGLE SERVICE ARTICLES MUST BE DISCARDED AFTER EVERY USE.NO REUSE OF SINGLE SERVICE ARTICLES.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.BACKFLOW PREVENTION DEVICE TO MOP SINK NOT INSTALLED.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST CLEAN FLOOR SURFACE IN DRY STORAGE AREA, UNDER AND BEHIND ALL EQUIPMENT

CLEAN THE EXTERIOR AND INTERIOR OVEN AND THE DOUBLE DOOR REACHIN FREEZER AND DOUBLE DOOR REACHIN COOLER.
REPLACE THE MISSING FLOOR TILES IN THE PREP AREA, SERVING LINE AND THE LUNCHROOM. CLEAN THE FLOORS IN THE PREP AREA AND STOREROOM ALONG THE WALLS AND IN THE CORNERS AND UNDER THE HEAVY EQUIPMENT AHS DIRT BUILDUP ON THEM.
CLEAN THE EAST WALL BEHIND THE OVEN HAS SPLASHED ON FOOD DEBRIS ON IT.
CLEAN ALL THE VENTS IN ALL THE BOYS AND GIRLS TOILET ROOMS HAS DUST AND DIRT BUILDUP ON THEM.
PROVIDE MISSING SNEEZE GUARDS AT THE SERVING LINE.
