Cleaning equipment shall be properly stored away from food, utensils, equipment, and clean linens. ORGANIZE ALL CLUTTER THROUGHOUT,i.e. IN REAR STORAGE AREA, OVER COOLERS, UNDER PREP TABLE, REMOVE ALL BROKEN/UNUSED EQUIPTMENT.
All utensils shall be thoroughly cleaned and sanitized after each usage. DETAIL CLEAN SHELVES THROUGHOUT, REMOVING ALL CARDBOARD ON SHELVES IN COOLERS.

No hand drying device or soap provided at exposed handsink. Instructed to provided soap and paper towel at exposed handsink for food handlers at all times. Critical citation issued 7-38-030.
'No Smoking' signs not posted or displayed on premises. Instructed that establishment must have 'No Smoking' conspicuously displayed in plain view of establishment customers. Serious citation issued 7-32-020(a).
Evidence of live insect activity found on premises. Observed live gnats-approximately 10 through-out rear prep area. Also, no pest control logbook maintained on premises. Instructed that establishment must obtain licensed pest control company and provided logbook with all required documents. Must remove all live activity. Serious citation issued 7-38-020.
Floor through-out premises not cleaned. Instructed to detail clean daily.
Unnecessary articles stored throguh-out premises. Instructed to remove all unused articles and elevate stock 6 inches or higher and away from wall.

Previous serious violation corrected 9-7-2012, report#1235116: Evidence of live gnat activity on premises. Observed approximately 30 gnats in rear of store near exit door. Instructed to detail clean and sanitize areas and have licensed pest control service premises and provided logbook with all required documents. Critical citation issued 7-42-090. Owner was able to minimize gnat activity on premises during inspection.
