All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED TEN MOUSE DROPPINGS ON FLOOR OF REAR STORAGE ROOM NEXT TO SPILLED CAT FOOD PELLETS, TWO DEAD RATS ON BASEMENT FLOOR, CAT FECES ON BASEMENT FLOOR IN THREE AREAS. NO LIVE CAT OBSERVED ON PREMISES AT THIS TIME. MUST CLEAN ALL AFFECTED AREAS. MUST ALSO PROVIDE A PEST CONTROL LOG BOOK WITH ALL NECESSARY DOCUMENTS AS REQUIRED. ESTABLISHMENT HAS RECEIPTS FROM HANSEN VERIFYING SERVICE BUT NO LOG BOOK AS REQUIRED.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO CITY OF CHICAGO FOOD SANITATION MANAGER WITH A VALID CERTIFICATE ON PREMISES AS REQUIRED WHILE SELLING FRESH MEAT AND SLICING OF READY TO EAT DELI MEAT AND CHEESE; MUST PROVIDE.
Smoking shall be prohibited in all public places except in designated smoking areas. OBSERVED A HALF SMOKED CIGARETTE IN REAR PREP AREA ON THREE COMP SINK AND A PACKAGE OF CIGARETTES ON REAR PREP TABLE; INSTRUCTED TO REMOVE. INSTRUCTED THAT ALL SMOKING MUST BE DONE OUTSIDE OF PREMISES FIFTEEN FEET FROM FRONT ENTRANCE DOOR AND EVIDENCE SUCH AS PACKAGES OF CIGARETTES AND HALF SMOKED CIGARETTES MUST BE REMOVED FROM PREP AREA.
