The walls and ceilings shall be in good repair and easily cleaned.OBSERVED WALLS BEHIND HANDICAP RAMP IN STUDENT DINNING PEELING PAINT AND ON SAME WALL PIPE HAS EXPOSED INSTULATION INSTRUCTED TO REPAIR.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.OBSERVED WATER STAIN IN REAR PREP KITCHEN LIGHT SHIELD INSTRUCTED TO CLEAN.

Outer openings not protected or rodent proofed. Southwest entry door-right side, right corners has 1/4 inch gap at bottom. Instructed to rodent proof door to prevent rodent and insect entry. Serious citation issued 7-38-020.
Utility sink not maintained in boiler room. Instructed to install drainline to properly drain waste water. Drain pipe in boiler room leaking. Instructed to repair to prevent leak.
Floor not cleaned in boiler and basement storage room not cleaned. Instructed to detail clean and remove excessive water on floor.

NOTED CLUTTER IN SUB-BASEMENT STORAGE ROOMS; MUST REMOVE UNNECESSARY ARTICLES, CLEAN AND ORGANIZE TO PREVENT PEST HARBORAGE. ARTICLES MUST BE STORED OFF FLOORS, AWAY FROM WALLS.
All utensils shall be thoroughly cleaned and sanitized after each usage. INSTRUCTED TO CLEAN/SANITIZE; MICROWAVE OVENS, CABINETS AND COOLERS INSIDE STAFF LUNCH ROOM AND JANITOR'S BREAKROOM.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST CLEAN FLOORS THRU-OUT; JANITOR'S BREAKROOM, SUB-BASEMENT STORAGE ROOMS AND BOILER ROOM, AND TO MAINTAIN FLOORS DRY AT SAME AREAS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO REPAIR LEAKINGS PIPES; UNDER 3PART SINK IN KITCHEN, IN BOILER ROOM AND IN STORAGE ROOM 1 IN SUB-BASEMENT. ALSO REPAIR FAUCETS IN 3RD FLOOR BOYS WASHROOM.
