I get sinus related migraines about 5x per month. I neti but it doesn't work but for a few minutes. Saw specialists and said nothing is wrong with me and they don't know why I get them so often. I will try the sinus rinse as opposed to the neti pot.
I've dealt with this for years and made lots of mistakes. I strongly, strongly encourage you to go to an allergist rather than an ENT first. I wish I'd have done this. Of course nasal blockage due to polyps might need an ENT to treat. But if you don't know what the problem is, I'd start with an allergist. The headache typically comes from sinus swelling, which can come from allergies.
Also, I would track the pollen and mold counts to see how you feel when certain counts are high. www.pollen.com

I've done the allergy drugs and the allergist route for plenty of years then in 1995 I found Pycnogenol and my allergy issues were gone in about 4 days...I took Pyc for the first year and then went to grape seed Extract and it's now 22+ yrs...no drugs, no allergist, no allergies/sinus issues. I'm 79, so started on the supp when I was about 57. It's great to do no drugs or doctors. J

I used to get terrible sinus headaches that took me out for days. Even after the headache eased, the stress of so much pain and the lack of sleep left me exhausted for days after the fact. There were nights when I'd just sit there and sob. Sometimes the pain was in my face and sometimes it was in one of my ears.
Got the sinuses roto-rooted a couple years ago as my issues were not from allergies, but from an excess of tissue in my sinuses. Air pressure changes would make the tissue swell and cause that unbearable pain. It was never consistent, but things like wind, extreme temperature changes, weather fronts and specifically storms could trigger it. I frequently used marijuana edibles to cope because I didn't want to get into opioids and the doctors didn't believe the pain was THAT bad.
I continued to get sinus headaches regularly after the surgery for several months, but at the one-year mark I realized I hadn't had one of those awful all night festivals of pain in quite some time. I think the tissue needed time to heal for the surgery to be effective.
I still get headaches, but some Advil PM and a hot shower solves the problem.
Adding this: The ENT was skeptical that surgery would fix the problem because he didn't think my sinuses had that much of a malformation, but he was willing to give it a try. So maybe your doctors are seeing anomalies but feel it isn't enough to mention?

I just had the MRI with contrast for a suspected brain tumor (no tumor found) but the radiologist found that even though I had sinus surgery in 2011, the mucosa in the maxillary, sphenoid and ethmoid sinuses continues to thicken. I think that's what's causing my non-stop sinus problems this year. I'll ask my ENT if surgery is an option -- not that I long for it.
