It kinda depends on where you are at that moment as to how you answer the "Where is NYC or Philadelphia" question, or any other "where is" question for that matter.

If I'm in Montana they are definitely east. If I'm in Virginia they are probably north. If I'm in Louisiana I would say they are northeast.

Yeah, it's funny, people on the West Coast routinely lump the Eastern seabord, incl. parts well inland, into "East Coast", which is kind of revealing in a way. Granted, my experience was in LA, where it almost seems there's a tendency to overlook everything outside of the northeast corridor as less important.

By the way, one of my favorite posters is the "Views of the World from X" series. If you look at the LA one, you see this shrunken US mainland mass with a big "New York" at one edge (showing what people on the West coast view as important in the East) then very little else of any significance. It perfectly captures how people really see the rest of the country out there. The NY version is probably something very similar but from the opposite perspective.

Btw, my guess RE: Philly is that if Jay Leno were to do one of his stop the average schlub on the street in LA and ask them where Philly is I think a lot of people out there would have no clue where it is and would guess it's a giant suburb of NYC that's just North of those other huge NY suburbs, Baltimore and DC.
