Missouri- St. Louis I think
New Jersey- Roselle and East and South Orange
North Carolina- Raleigh
Ohio- Columbus and Cleveland
Pennsylvania- Philly

As a Canadian and seeing what people outside the US view Ohio I'm surprised by Cinncinati, Frankly outside the US all people associate with it is WKRP.
They may now be similar in size but Cleveland would be the answer............no questions asked.
I'm not trying to offend but outside the US Cin is a radio station, Columbus is an explorer, and Cleveland is Ohio.

Outside the US, the United States is New York, Miami, Detroit, and Hollywood. Foreigners are as unfamiliar with the states as we are with their provinces.

Could not be further from the truth. I've spent considerable time in Europe and you'd be surprised how much they know about US cities (I guess that plays into 'Hollywood' as you mentioned)
Now as far as US states, then I'm sure they aren't as versed (but a lot of Americans aren't either for some strange reason)
