Any city where you can find a job paying more than minimum wage is good. Wilmington, NC and Charleston, SC are the "destination" cities that will best environment.
Not NC, but Hampton Roads area of Va (Norfolk/Va Beach).
Agreed. If they are seeking to find professional jobs, Hampton Roads is better. If they just want to party and live on service jobs, I'd suggest Myrtle Beach, SC.
Charleston offers the best combination of restaurants (nationally regarded culinary scene), shopping, entertainment, culture, and jobs easily. This is where the OP should start her search.
I lived in Mt Pleasant for decades. loved the area, still miss it. But do be aware it is not cheap. And traffic can be brutal getting into and out of Charleston proper during rush hours.
If Charleston is too large for you, Mt Pleasant is very nice. You might also consider the Hilton Head, SC area, Beaufort or Georgetown, SC. These have many retirees and may be too quiet for you. Myrtle Beach may be too much of a tourist destination to support many occupational therapy jobs, but who knows?
Other than Wilmington, in North Carolina, you might look at Hampstead, Jacksonville, Swansboro, Beaufort, NC or Greenville. There's also a Veterans Administration Hospital in Fayetteville that might employ OT's.
