What's in his ear with the cord? Since I am a child of the 60s is that earphones for a transistor radio?

That really is a charming photo. Whether Ron Paul wins or not, he still has his heart in the right place (on most issues anyhow ).

That's an earpiece for a portable transistor radio. It only broadcasted in mono, probably at that time only AM band radio.
