The Rust Belt cities which still haven't fully turned around, and the rural areas down south and out west.

I think heroin and crack are city/suburb drugs and meth is something in the country

Everywhere. Boston and Providence have an opiate/heroin epidemic 2-3 times worse than the rest of the US based of deaths per 100k

Also in a lot of the yet to gentrify boston neighborhoods you just can feel the methadone clinic vibe...the crackheads are down from ten years ago but some areas in the inner city have a major drugged out vibe.

^^^ This, rather than many of the failed guesses and stereotypical examples provided thus far on the thread. It would appear more rural, cold northern areas of the country are dealing with a significantly higher percentage of drug use than the alleged southern border states.
