Niacin ER 500mg (generic for Niaspan) is a tier 2 ($8) copay for BCBS but United is keeping it at tier 3 ($40). United only shows Niaspan (brand) even though they will supply the generic (Niacin ER) at the same price. My wife warned be not to leave BCBS but I didn't listen and now this year I will have to pay $32 extra a month. We will both be going back to BCBS Alabama part D next year.

Formularies change year to year. It's wise to keep an eye on them. Just check out the formulary before you switch back. I'm surprised that the generic form is tier 3 though.

Have you checked their formulary for a substitute drug that may be as good?
