It means the patient had inflammation of the heart muscle due to either environmental chemicals or drugs (toxins are poisons).
They also had dense fibrous tissue surrounding the heart (the pericardium is the tissue that encloses/surrounds the heart).

Is there a common name for this? Or, is it so general, we really can't say. Is it a cause of death that could have several common causes?

It means the person likely died of heart failure

Yeah, like I said, "heart failure".
The post lobectomy ect is just the location of the adhesions in the pericardia I believe.
Was this someone you knew plmokn?

I looked up "pericardia" and it is something to do with " the fibroserous sac enclosing the heart and the roots of the great vessels"
It was my mom's first husband (not my dad). I came across his death certificate.

The guy was only 26 years old. This was 1951. He had TB. (Picked it up in a POW camp in Germany.)
Might TB be related to the cause of death? TB is lungs though. That's why this is confusing to me.
