Another one is The Fly with Jeff Goldblum. I got to the part where he started to pull his fingernails off, gagged twice, and gave up.

Marley and me (can't stand the euthanasia part)
Vertigo (great movie, but it gave me a lasting set of the creeps)

Seeing Old Yeller get shot broke my heart as a kid, I've never wanted to watch that again...and haven't.
