Hi folks. I need some advice. For a year I have been trying to get my documentation together. After a major depressive episode I'm better unfortunately during that time i let my documentation and life fall apart.
I got my life mostly together. This is what i have so far.
Social Security Card
Birth Certificate (original no copy)
prepaid debt card
pay stub from a job i did with a client
Even though I don't need a home phone, I got one to get the utility bill.
I've tried getting my passport to get this done, it was denied due to lack of documentation including bringing in an identifying witness and the above documents.
I called 311 and a very nice lady did the best she could. She went through the list step by step. I'm not divorced or were ever married. I don't carry a gun. I was never in prison. I don't have my tax forms yet because I had no income for 2013 (nor 2010 through 2012) I have no health insurance, I cant afford it yet.
I was never on public assistance. I don't have a bank statement because I can not open up one without a government issued picture id.
If I go to DMV, what can I do? I can't live normally because of this, and it is affecting my social life and work every day. From The Dept of State to passport/311. I really am getting discouraged and it's affecting my depression not even my meds are helping. I'm desperate, I don't know what do do. I'm in danger of losing everything I've worked so hard this year to get back. Thank you.
I've been through all of the above suggestions. Every single document that I don't have I do not qualify for. In some cases I don't qualify for them without a NYS ID. Is there anything I else I can do beyond what is on DMV's list? I'm not asking for special treatment but I would imagine this is something the state of new york would want me to have.
Passport is the problem that you seem to have. That will be more difficult to get even with the state ID once you do get that. The stuff you have like birth cert. & SS card should be what you need to get state picture ID (which u need for passport anyway) For the state ID it looks like what they are looking for is something that proves residency. Like something with your address that is mailed to you. Like utility,CC. Even SS statements are acceptable. Think you do need to prove that you actually have a physical address where they can always contact you to get state pic ID.
None of the items you have listed are valid for proof of identity. You need W2 forms that are mailed to you individual pay stubs are useless for this. You will never get a passport without getting the proper ID first. Imagine what would happen if were made easier for anyone to get what you are looking for?
Do you have a baptismal record? Even if it's written in a Bible it's still proof and the DMV takes that.
Don't dispare, there's an answer to everything. You're hanging too much on getting ID but you can still function in the meantime. It might be a pain in the arse but even cashing your check can be done, say at a check cashing place where all they'll need is an utility bill to verify your name and address.
Do you have a company id? Banks usually will take that.
To get a NYS ID you need proof of name and proof of DOB. You can prove those with your SS card, and your birth certificate. PLUS if you use your SS card as proof of name it means you only need to show 4 additional points of ID.
What you have:

Utility Bill- 1 point
Did you graduate high school? A high school diploma counts as 1 point. If you call the school you attended you can pick up/have them mail you proof of graduation. It's really easy to get- I've had to do it before.
Do you have any prior W2 forms? The list doesn't specify that the W2 has to be from last year. If you filed your taxes with a company (i.e. HR Block) they keep copies of your W2 on their website that you can access for a couple of years after filing. That counts as 1 point.
Does your prepaid visa have your name printed on it? If so that counts as 1 point too.

I don't have a w2 form, I'm an independent contractor I only have w9 forms. it doesn't say w9 on DMV's site.
