When we have guests from out of town we always go touring around. I love allowing my friends and fam to get an inside's view of our lovely old city. Likewise, we always ask our hosts what they enjoy seeing and doing in their home towns when we go a'visiting. if no one is inclined, then we go out on our own, for a little while.
If it's a regular family visit in familiar settings we may fall into a domestic pattern, but it's usually punctuated by some excursion or other.
When I visit out of town relatives, I tend to wonder around a lot. I like to go for a walk or ask if they would like to join me for a walk in the park. If I am staying overnight, I give them a chance to gather themselves for a few hours and I just go to the local mall or walk around town for a few hours by myself.
When relatives are visiting me, I offer them a choice as to daily activities and ask them what they like to do. I don't like being a tour guide, but I'll at least give them an option as to some outdoor activity, unless they like to stay indoors. That's cool, too.
