I believe that unless there is a VERY good (i.e., health) reason, denying your spouse sex is CHEATING on your spouse, every bit as much as infidelity is cheating.
When atrophy occurs from the lack of hormones required to support a uterus, any sexual activity becomes extremely painful.
Usually uterine atrophy doesn't occur until long after menopause, well into a woman's 60s, unless it's partnered with endometrial cancer. Usually the only women who shouldn't are those with a history or high risk of breast, ovarian, or uterine cancer. And there are a number of non-hormonal products that can help restore some moisture.
I don't say this lightly, but if uterine atrophy is causing your spouse pain, has she thought about having a hysterectomy? That would get rid of the pain, and not having pain (and therefore not fearing pain) can be a tremendous help in getting a sex life back. I realize it's a tough decision, though.

Sex goes south in 90% of all marriages. The answer is simple, if you like good quality sex, don't get married! If your desire to reproduce is great get married and forget about mind blowing sex!
