It's kind of hard to pull off most women aren't open to it or want bothered and there's not really many ways to start it up. They either have to be at the water fountain the same time as you, be on a machine beside you or waiting for the one you're, at the towel dispenser, etc. The options are pretty limited.
It totally depends on the gym and the area the one I'm going to now seems to have more friendly than most I've been to in the past and it has a good crowd. I started some small talk with one that was on a machine by me a couple of weeks ago to lay a foundation. Then I haven't seen her since so I haven't been able to initiate stage 2. It was around the holiday season though people are always friendlier around that time of year.

the point of that is to be so totally awesome that they tell you that without you having to solicit anything
If you have to solicit it, you're not that great
They can see it when I flex in the mirror and you can see all the striations and veins in the upper arms/shoulders area

I wear headphones to the gym to keep people from talking to me. I've had guys push to talk and I hate it.
The gym is not a place to pick-up women IMO.

You'd think, but it doesn't always mean they don't want people to talk to them. Sometimes, they wear them and listen to music because they're bored and have nobody to talk to.
Headphones are the universal symbol for "Do not engage."
No headphones? Smile and and make eye contact, then after a couple of times, just take the leap.
just about everybody wears headphones. I don't think you can assume everybody is unapproachable
Hell I probably look incredibly unapproachable. I'm usually wearing a Slayer or Metallica shirt, big overhead headphones, hat and I work out with a ridiculous intensity and probably look very pissed off to outside observers (the key to ferocious work out intensity is rage)

I don't do steroids so I don't have that gross look...I still look healthy and athletic
I'm 5'10 180...nothing crazy. I'm probably about 10 lbs bigger than your typical "ideal beach body".
I might be the only guy who thinks this but I would never approach a woman at the gym. People go there to workout not to be hit on at least I wouldn't want anyone bothering me while I'm working out.
