Many women prefer NOT to have obstetricians.... rural or urban.
Your less likely to have a C-section with a midwife. OB-GYNs are notorious for being overbooked and unable to be available when moms give birth..... or they're out on the golf course so much that they insist on doing their work on their schedule rather than the moms and babies.
I'd take a good master's prepared midwife over many of the OBs I've come in contact iwth over the years - in a heartbeat.
There are many, both men and women, who prefer to consult clerks in health food/pill stores. There are others who go to practitioners who burn various plants and saturate the area with what they say are healing fumes. They seem to go further and further down the medical hierarchy looking for the perfect healer. They delude themselves because that perfect healer doesn't exist. They eschew the best available simply because of fear.
Nutrition is as important to the health of the elderly as it is to others. Yet so many in cities are apparently unable to supply themselves with fresh produce and, in some cases, without even frozen food. Canned food is available online, but people often wish a fresh salad or real mashed potatoes. Many people desire fresh meat as well.
I don't think the trend has anything to do with the ACA - that is insurance based regulation.

Here's a cluestick.... there are VERY FEW hospitals in rural areas. Getting rid of the ACA will close the doors of the few that are left.
Americans call it OBAMACARE. We have been forced to pay for Medicare for years, only to see Medicare payment cuts to hospitals with the advent of Obamacare. We don't need "Repeal and Replace." We just need "Repeal."

It IS true. In the past rural hospitals were partially supported by government programs aimed at encouraging improved access to health care in rural areas. Most, if not all, of those programs have recently been gutted.
The ACA allowed such hospitals to continue because the ACA provided an alternate revenue stream.
Once both the federal funding AND the revenue stream from ACA patients is gone, hospitals will have to close or drastically reduce personnel and services because money to cover salaries and operating costs doesn't magically appear.
I've lived in rural areas - also worked in a rural development hospital - so I am VERY well aware of how this all works.

I'll support "repeal" - as long as we institute single payer coverage (aka "socialized medicine") first.
