Seeing that Magnum PI is coming back, I'm thinking about prominent TV characters that are never seen or heard.
Several are heard but not (or barely ) seen, like Robin Masters of Magnum PI. (We heard his Orson Wells voice.)
Charlie Townsend of Charlie's Angels. Carlton the Doorman of Rhoda. (Did we see his hands?) Debbie Wolowitz of Big Band Theory. (We saw her pass quickly behind a door once, I remember.)
But the only character never seen or heard that I've come up with is Lars Lindstrom of The Mary Tyler Moore Show. He was always around, and Phyllis talked about him a lot, but we never saw or heard him.(Unfortunately he died before the Phyllis show started.)
Wilson from Home Improvement Maris from Frasier
Norm's wife Vera from Cheers
This immediately came to mind. The source of hundreds of jokes and scenes over the years, but we never caught a wisp of her.
Frasier: "Must say Niles, that is a striking tie you're wearing"
Niles: "It was a gift from Maris. She had this made for me to commemorate the end of that dreadful winter when she had that obesity scare"
Fraiser: "Ah yes, I remember her struggle to lose that holiday pound"
(Niles is suffering from narcolepsy as a result of stress from his divorce with Maris):
Frasier: Niles! (slaps him on the back to wake him up.
Niles: Did I do it again?!
Frasier: Yes. You fell asleep with your cheek right against the ice tray.
Niles: Oh that's so strange. I dreamt I was tangoing with Maris.
Niles talking about the idea of freezing some of Maris' eggs in case the two of them ever have a kid someday. Frasier (again): "Oh, I suspect they're just a few degrees away from that now."
Captain Tuttle from "M*A*S*H" albeit it's only one episode. It's a little bit different than the others in that he does not exist, but only Hawkeye, Trapper and Radar know this. Everyone else thinks he's real

Mrs. Columbo was talked about a great deal on "Columbo" but never seen nor heard on that show. She did manage to materialize in the form of a short lived "Mrs. Columbo" series, but that was separate from the original show.
Hawkeye Pierce talked about his father back in Crab Apple Cove, Maine, and sometimes we saw Hawkeye talking to him on the phone, but we never saw nor heard the senior Mr. Pierce, nor did we meet Colonel Potter's wife or Radar's Mom on MASH (Mrs. Potter, like Mrs. Columbo, did come into being on the short lived "After-MASH", and we once saw a photo on MASH of Mrs. O'Reilly, which was Gary Burghoff in a wig and makeup.
Radar O’Reilly’s mom (Gary Burghoff in drag) did appear in one M*A*S*H episode, seen in a home movie with other of his family members. Same with Frank Burns’s wife, as well as Peg, BJ Hunnicutt’s wife.
On US television:
Al’s mom on “Home Improvement.”
Peg’s mother from “Married...with Children.”
Margaret’s husband Lewis on “Becker.”
Karen’s husband Stan on “Will and Grace.”
Steve Urkel’s parents on “Family Matters.”
Most adults on the various “Peanuts” specials, usually with a wah-wah trombone as sound stand-in.
On UK television:
Liz’s husband on “Keeping Up Appearances,” who’s always off on business.
Mrs. Slocombe’s friend Mrs. Axelby on “Are You Being Served.”
Howards mom on The Big Bang Theory. We would hear her and catch "shadows' of her, but that was it.
That 70's Show. I could be wrong and they might be there somewhere, but most of the kids parents never appear, but they are occasionally talked about or seen being talked to on the phone.
The others I think of right away have already been mentioned.
