Assuming you have sick leave and vacation time, you simply try to get them in the morning or afternoon and come in late of leave early due to appointments. Most employers do not require proof for sick leave, but also don't object to using vacation time for non-medical appointments. if a really good candidate asked we'd agree to an after hours interview, but in most cases we'd give the times available and they would be 8am-4pm.
