I don't know what should be the look and feel of 1st world, since the european, american and developed asian cities look nothing alike. That city causes me a impression to be generic. It could be located anywhere in the world.

Have you been to Penang? The traffic is no different to that of Sydney. It is nothing like Vietnam, as an example. Everyone drives in their lane and observes the road rules. Malaysia is a middle income country on the world scale. It is an excellent place to travel, as an aside.
George Town, Penang is a charming city and popular with foreign including Western retirees. What problems it has are more First world issues and rather a stand out place in the region.
As stated above, Malaysia in general is an easy and pleasant place to travel with a lot of western trappings making it a perfect introduction to Asia for the less 'adventuress' .
