Observation: Pizza cutter stored on cloth in between use. In use pizza spatula stored on common surface that is not cleaned on a regular basis (cash register area). Utensil used to pull pizzas out of the oven and place in the pizza boxes is stored on a dirty surface on top of the oven in between uses.
- Physical facilities maintained clean: Cleaning frequency, restrictions, and methods
Observation: Floors under equipment throughout the establishment including in the walk-in cooler have a build up of dust, flour, and residue. Ceiling tiles in particular around the vent hood are soiled with dust and debri. Vent hood surfaces above the oven are soiled with dust and debri.
- Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
Observation: Surfaces throughout the kitchen areas soiled with food residue including on top of ovens and exteriors of ingredient bins.
- Common name-working containers
Observation: Spray bottles containing chemicals not labeled with name of the chemical. Spray bottles were labeled at the time of inspection. Spray bottle labeled "pee" that actually contained sanitizer according to an employee was dumped out. Unlabeled bottle of glue and container of soap. Both were labeled properly during the inspection.

Observation: Ingredients stored outside of their original containers in unlabeled containers.
- Separation from food, equipment, utensils, linens, and single service
Observation: Chemical spray bottle stored with disposable plates. Spray bottle was moved to an area away from food and food contact surfaces at the time of inspection.

Observation: Used wiping clothes stored on food prep tables.

Observation: Light bulbs in food prep areas without protective shielding.
- Food employees hair is effectively restrained
Observation: Food handler with a beard was not wearing a beard restraint.
