Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.
Critical: FOOD CONTACT SURFACES - WAREWASHING EQUIPMENT, CHEMICAL, TEMPERATURE, Ph, CONCENTRATION AND HARDNESS Observation: The bar warewash low temp machine was not sanitizing properly. Tested and found to be at 0 PPM. Sanitizer is not being used according to EPA registered label use instructions. FCS sanitizing bucket solutions with chlorine found at well over 200 PPM.
Correction: Provide training to employees on proper use and concentration of sanitizer or repair or adjust warewashing equipment to provide proper sanitizer concentration. Solutions remixed for FCS buckets. Warewash for bar area replaced during a later follow up visit. Working correctly now.
PACKAGED AND UNPACKAGED FOOD - COVERED CONTAINERS OR WRAPPING Observation: Foods in the Americana freezer were observed to be unwrapped or uncovered.
Correction: All foods that are not part of a cooling process shall be covered at all times to prevent cross-contamination. If cross-contamination is evident, discard suspected foods.
FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR Observation: The raw potatoes in the walk in cooler are stored on the floor in the back of the walk in cooler.
Correction: Store all food items 6 inches above the floor.
THERMOMETERS - FUNCTIONALITY - WARMEST PART OF UNIT Observation: Thermometer in the True 1dr cooler was missing
Correction: Place a thermometer so that it is located in the warmest part of the refrigeration unit.

Critical: PACKAGED AND UNPACKAGED FOOD - RAW OVER RTE-FOODS (corrected on site) Observation: There was ready-to-eat soup stored under raw eggs in the walk-in cooler
Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination.
Critical: SEWAGE - BLACKFLOW PREVENTION Observation: A direct connection exists between the sewage system and a drain from ice machine and ice bin. An air gap must be present not an air break.
Correction: Design the sewage system in a manner that would preclude a direct connection between the sewage system and the drain from which food is placed. Remove the direct connection.
Critical: TOXIC SUBSTANCES - SEPARATION (corrected on site) Observation: Spray bottles of chemicals were stored hanging on a rack above food products
Correction: Rearrange area so that toxic materials are stored away from food, equipment and single service items by spacing or partitioning.
Critical: TOXIC SUBSTANCES - CONTAINER PROHIBITION (corrected on site) Observation: Liquid butter was stored in a container previously used for dish soap.
Correction: Discard chemical containers after use and discard any food found stored in these containers.
IN-USE UTENSILS, BETWEEN USE STORAGE Observation: The bar ice scoop was stored with the handle in contact with the ice.
Correction: Store in-use utensils in the food item with handle extended, on a clean surface, running dipper well or in a container of water greater than 135°F.
LIGHT BULBS - PROTECTIVE SHIELDING Observation: Lights located in kitchen are not shielded. Repair the damaged shields and clean the existing ones.
Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.
