KDA_3_20111B - --There were 10 glass jars of home canned items sitting on the shelf in the kitchen pantry. COS. PIC removed items from kitchen use. Educated staff that only jellies and jams area allowed in the FE.
KDA_4_60111C - --The microwave has an accumulation of dried food debris on the interior walls and ceiling. The mixer has an accumulation of dried food debris on the base. The shelving units have dried food debris where clean dishes area being stored.
KDA_4_60211E4 - --There is visible pink and black mold on the guard plate in the ice machine.
KDA_4_70211 - --The in use dish machine did not have any sanitizer. Test strip for chlorine was 0. Discovered that bucket was empty. One load of dishes was washed. COS. Bucket was replaced and chlorine test was 100ppm. Dishes were rewashed.
KDA_4_90311C - --Located at the self service food buffet line, there were 3 containers of eating utensils being stored with the eating end up and consumers could grab the utensil and touch the other utensils with bare hands.
KDA_5_20515B - --The dish machine has a leak under it and a bucket is on the floor to catch the dripping water. The hand sink faucet is broken in the bar area.

KDA_6_50112B - --The top of the dish machine and shelving for dry clean dishes, have an accumulation of aged dried rodent feces.
KDA_7_10211 - --There were 3 spray bottles of clear liquid and 1 spray bottle of blue liquid with no label. PIC said blue liquid was window cleaner, 2 of the clear liquid were vinegar/water, and 1 bottle of the clear liquid was bleach water. COS. Bottles were labeled.
KDA_7_20111A - --There was one roll of single use paper towels sitting on the shelf next to and touching one package of mouse killer. COS. Towels were moved.
