KDA_3_30212 - --On a metal table in the Bakery area there was a working green plastic spray bottle with clear liquid in it and not labeled. An employee said it was water. COS-labeled the spray bottle.
KDA_3_30511A1 - --In the back food storage area there was one box of weight loss food products being stored directly on the floor. COS-an employee removed the box off of the floor.
KDA_3_50118A1 - --In the Deli RIC 2 there was an open package of hard salami with a sell by date of 12/6/15 (held past 7 days) and it requires refrigeration. COS-discarded.
KDA_4_60111C - --There is dust build up on the 3 sets of fan blowers in the Dairy WIC.
KDA_5_20515B - --The atmospheric vacuum breaker on the mop sink located by the Produce area leaks when water is supplied. There is a Vented-Tee valve connected directly to the faucet and one of the hoses is connected to the chemical dispenser which has it's own backflow preventive device. The hose was also above the flood rim. The atmospheric vacuum breaker on the mop sink located on the southwest wall leaks when water is supplied. The hose was hung up above the flood rim. The turn knob on the faucet at the 3 compartment sink located in the Produce room leaks when water is supplied.
KDA_6_501112 - --In a tamper resistant bait box by the back exit door, there were 2 dead rodents in the trap.
KDA_6_50112A - --There is ice build up on the ceiling of the large walk in freezer.
