  • 6.2 Repair or replace the non-functioning light fixtures throughout to provide lighting as specified in OAC 3717.
  • 3717.43 Provide a copy of the food license on location at all times. This inspector requested a duplicate license to be mailed to this location per PIC request. Original license was mailed from this office on 2/22/12.
  • 3.2 Label the bulk container on the storage rack with the name of the food, to avoid confusion and misuse.
  • 5.1 Repair the mop sink faucet. Reportedly the faucet will be removed in the near future but the floor sink will remain for the disposal of mop water.
  • 4.5 Observed ice accumulation on the walls, ceiling, shelf and on the condenser line of the walk-in freezer. Clean this area.
  • 4.4 Observed droplets of water pooling on the ceiling of the walk-in cooler seams and leaking on to the floor. Repair this area to prevent contamination of food products.
  • Reportedly operation is preparing foods at 8AM daily.

-Observed employees wearing single-use gloves to prevent bare hand contact.
- food were holding at proper hot and cold holding temperatures during this inspection.
  • 4.4 Observed the reach-in cooler door (drive-thru side) in disrepair. Repair this in order to keep equipment in good repair.
  • 6.4 Observed pooling water under the drive-thru iced tea area. Clean this area and make any necessary repairs.
  • 6.4 Observed missing grout in several areas of the floor tile. Repair this in order to provide a more easily cleanable surface.
  • 6.2 Replace the burned out light in the walk-in freezer in order to provide adequate lighting.
 11/2/2011Standard2HANDWASHING: Observed a wiping cloth bucket stored on and blocking the front hand sink. Maintain all hand sinks easily accessible at all times in order to facilitate proper hand washing. PIC relocated the bucket today.
-PIC knowledgeable of proper cooling and reheating procedures.
-Proper time-in-lieu procedures observed for potatoes.

  • 4.5 Clean the walk-in cooler door seal in order to remove black residue.
  • 6.4 The following areas need cleaned more thoroughly:
  • -R/dusty vent cover above the hot holding tables
  • -debris on floor under dry storage shelving
  • Maintain physical facilities clean.
