    Observed the floor tile is torn, ripped and or missing in different locations through out the kitchen and on the cooks line Leaving raw exposed concrete. Floor repairman has been called and repairs are expected to complete by the 15th of Nov.

    Observed the ambient temp of the sandwich unit on the cooks line is operating at 48* all cheese, lettuce and dressing where discarded approx. 10lbs

    Observed the surface inside of the microwave is peeling and chipping. Microwave is not allowed to be used.

    Food facility does not have adequate refrigeration equipment / capacity to maintain all refrigerated foods at a temperature of 41 degrees F or below.

    Observed the inside of the bulk ice machine has black slimy residue, machine was shut down and ice was dumped, unit is not allowed to be used until it has been cleaned and sanitized. All ice at waitress station and bar was also dumped.

    Obsevred mouse droppings on the shelf in the storeroom behind the kitchen. Obsered mouse droppings underneath of the reach in freezer ouside of the cooks line.

    Observed a dead mouse on a glue board in the upstairs storeroom.

    Observed the internal temp of product inside of the 2 door reach in cooler on the line was 54* ambinet and the internal temp was 54*. Observed the ambient temp of the sandwich unit on the line was 63* and the internal temp of deli meats cheese cut tomatoes etc was 57*. Observed the internal temp of the filet and cheese in the drawer underneath of the broiler was 54*. All product in cooling cases listed above was shut down and product was discarded approx 55lbs. The estab will be maintaining a temp log with ambient and internal temps of product every hour. Repairman has been called and is on his way.

    Observed there was old mouse dropping in the dry storerooms to numerous to count. The dropping where on the shelf and along the floor and inside of box of plastic lids. Black light was used. All cans and plastic containers in storeroom need to be cleaned and sanitized, all droppings throughout need to be cleaned and sanitized. An attempt was made in the storerooms, however not all product was pulled from the storeroom. Observed there is still droppings in and on the some of the flat ware and on the edges of the shelfves.

    Observed clean bar glasses stored inside of the ice bin at the bar, the ice was not being used for consumption, the ice was being used as a coolant for the glasses. Employees hands come in contact with ice and glasses.

    Food facility does not have adequate refrigeration equipment / capacity to maintain all refrigerated foods at a temperature of 41 degrees F or below.

    Observed the sandwich unit on the cooks line has a ambient temp of 61* and the internal temp of the various product throughout the cooler was 58*. cut tomatoes, cheese, turkey, ham, various dressings. all was discarded approx 35lbs.
  • Cleanability of Multiuse Food-Contact Surfaces & CIP equipment (46.542)
    Observed there was duct tape on the handle of the fryer basket which was in the fryer.
  • Objective of Cleaning Food Contact Surfaces & Utensils (46.711(a))
    Observed the inside of the hood canopy has grease dripping. Observed the blade for the slicer has old encrusted food. Observed the underside of the counter top mixer has old encrusted food. Observed the inside of the bulk ice machine has black residue. Machine was shut down and ice was dumped. Observed the soda gun and holder at the bar has a build up of slimy residue.
  • Repair & Cleaning Various Premises, Structures, Attachments & Fixtures (46.981(a)-(c)(g)(n))
    Observed the wall behind the dishmachine has a heavy build up of mold. Observed the floor in the bar and throughtout the kitchen prep has old food and garbage.
