    One dead roachlike insect was found on top of shelf directly above True freezer on right side when entering kitchen. One live roach walkin on kitchen floor. Two egg capsules in one slide out tray below kitchen stove. These were smashed. One live small roach like insect on top of shelf in food storage room. One roachlike insect on its back kicking on threshold going into walkin cooler. One live roachlike insect on glue board inside hot buffet cabinet.

    One live small roachlike insect was on top of customer table in seating area. Insect was killed. Two live adult roachlike insects were inside wall bracket that holds takeout paper bags. Insects were killed. One dead roach was found on lower shelf of front counter. Pesticide company was called and retreatment will occur August 23.

    Numerous (approx.90)dead roachlike insects on glue boards placed under kitchen coolers and prep table. One live roachlike insect inside conduit panel. Two dead roachlike insects on top of electric water heater. One live roachlike insect on shelf of counter below cash register.
